Complete the form below and based on your child’s age, MUW will send you your first Ages & Stages Questionnaire (ASQ). After completing the ASQ you will receive information on how your child is growing and learning, how you can support your child at home, and a free children’s book. If your child needs some encouragement, we will connect you to people and services who can help. In 3-6 months, you will receive a new age-appropriate questionnaire! It is quick, easy, and fun!
